Ketamine is a parenterally administered non barbiturate anaesthetic agent, in use for more than a decade. Recently ketamine use in psychiatry has increased for different indication. Ketamine works on NMDA receptor at PCP site. The mechanism of action is frequently called as dissociative anesthesia. In patient who are anxious and have subcounscious conflict because of various psychological issue and are not able to understand the conflict, this dissociative action of ketamine gives a solution. Patient is properly investigated before starting on treatment and then shifted to ICU for procedure. Patient is allowed to settle down. The nursing staff then puts an iv line and ketamine infusion is started at adequate dosing and slowly. Patient is engaged in conversation with the professional and initially a good rapport is established. During the session patient vitals are continuously monitored. During the session patient is continuously allowed to speak and various cues are given depending upon the history. Gradually patient loses inhibition and starts talking about the conflict in unconscious. The interviwer makes note of the interview.
Depending upon the patient patient sometimes is not able to dissociate and may not be able to resolve the conflict. However the dose of medication produces a rapid calming effect and also helps in patient with active suicidal thoughts. The depressive symptoms also improve with the treatment.
